If you are interested in enrolling:
- Please print and fill out the enrollment form and:
- Send it to school with your child
- OR it can be sent directly to Mary Simmons, 600 11th Ave, Fulton, Il 61252
- OR it can scanned and attached in an email: 5mctab@mchsi.com.
- Orders/payment MUST be at school before 9 am on order day, or sent directly to Mary Simmons BEFORE order day.
- Late orders will NOT be processed until the next order date.
